- Retailer Device OS version
- Device data signal strength (how many bars out of 5) and type (2G, 3G, 4G, WiFi, etc)
- YC app version
- Description of issue
- Steps to reproduce issue
- Screenshot/video recording of issue
- Timestamp of when issue occurred
